Uw Zorg Online 4.4.5

In collaboration with your healthcare provider, the Uw Zorg Online app grants you direct access to your medication overview as known by your general practitioner, enabling you to reorder previously prescribed medications, schedule appointments, and initiate an eConsult!

A list of affiliated practices can be found within the app.

We welcome your suggestions and feedback. You can provide these to us via the feedback button in the app or by email at [email protected].

Before you can begin, your practice needs to make the service available for you:

1. Download the app
2. Find your practice
3. If you already have an account for your GP's patient portal, log in with those details (skip to step 4).

If you don't have an account yet, request one by clicking the 'Sign up' button and filling in all necessary information.

After your request is verified by our practice -which may take some time- your account will be created and you will receive your login credentials via email.

1. Upon logging into the app, you will receive a one-time verification code via email or SMS that you need to enter
2. Finally, set a 5-digit pin code within the app to secure access
3. You can now start using the service

The functionality includes:

• Viewing your current medication profile as known by your GP
• Requesting repeat prescriptions directly from your medication list and receiving reminders when you need a refill
• Directly asking medical questions to your doctor through an eConsult and receiving a response once your consultation has been answered. Please note! eConsult is not intended for urgent matters or life-threatening situations. If you are unsure about the seriousness of your condition, always contact your GP by phone
• Viewing available slots in your doctor's schedule and booking an appointment at your convenience. You must also state the reason for your appointment
• Finding the address details, contact information, and opening hours of your doctor in the app. You will also find a link to your doctor's website

Privacy:

The app allows you to securely retrieve your medication data from the practice system and communicate with your doctor. Before activation, your identity is verified by the practice, and you receive a verification code to activate the app. Furthermore, you can secure the app with a personal 5-digit pin code.
